Touching Hearts at Home of Southwest Boston is seeking a Client Care Specialist with a customer service focus to help us continue to ensure that we provide quality home care for seniors in need. We are looking for a senior care professional who has case management, social work or clinical liaison experience in either a senior care community or home-based setting. Position Overview: 
The Client Care Specialist plays a pivotal role in ensuring the highest quality of care for clients and providing training, support, and oversight for caregivers within a home care agency. This position focuses on supervising client care, enhancing caregiver performance, and maintaining operational excellence. 
 
Key Responsibilities: 
  • Client Care Coordination:
    • Ensure all client care plans are implemented effectively and reviewed regularly for any necessary adjustments.
    • Perform periodic home visits to monitor service delivery and client satisfaction.
  • Coaching and Mentoring: 
    • Provide mentorship and support to caregivers, addressing performance issues and recognizing achievements. 
    • Support in the coordination of caregiver schedules and ensure proper staffing for all client needs. 
  • Caregiver Training and Development: 
    • Conduct orientation and ongoing training programs for caregivers, including skills development, safety protocols, and best practices in client care. 
    • Keep caregivers updated on agency policies, industry standards, and regulatory requirements in conjunction with agency ownership. 
  • Field Visits and Oversight: 
    • Conduct field visits to observe caregiver performance and provide in-the-moment coaching, training and support. 
    • Ensure all caregiving staff follow proper documentation procedures (written and digital care logs, expense logs, medication logs, etc.). Seek out opportunities to improve tracking protocols. 
  • Jump and Run Caregiver:
    • Act as jump and run caregiver n the event that there is a last-minute call off for a client who needs support.
Qualifications:
  • Proven experience in a caregiving or supervisory role within a home care or healthcare setting.
  • Strong knowledge of caregiving best practices, including patient safety, personal care, and emotional support.
  • Excellent communication, and interpersonal skills.
  • Ability to handle challenging situations with professionalism and empathy.
  • Valid driver’s license and an insured vehicle. 
  • **Please note: Touching Hearts at Home does NOT currently have health insurance or 401Ks. As we grow, these are benefits that we will explore, but not something currently offered.**
Compensation: $60,000.00 - $65,000.00 per year




Each Touching Hearts at Home franchise is independently owned and operated. Your application will go directly to the management of the franchisee where all hiring decisions will be made.
